Short-term support to help you recover, regain confidence and stay independent

Capecare’s Restorative Care service provides short-term, practical support to help older people remain safe and independent at home.

You may benefit from restorative care following an illness, injury, hospital stay or change in health or mobility. Support is based on what matters to you and the everyday activities you would like to regain, maintain or manage more confidently.

You do not need to be an existing Capecare Community client to receive restorative care.

What is restorative care?

Restorative care is short-term support designed to help you improve or maintain your ability to complete everyday activities.

Rather than doing everything for you, our team works alongside you to build your strength, skills and confidence wherever possible. Your support is guided by your individual needs, preferences and recovery goals.

This may include helping you:

  • recover safely following illness, injury or a hospital stay
  • improve your strength, balance or mobility
  • manage personal care more independently
  • safely use or manage equipment, braces or mobility aids
  • regain confidence completing everyday tasks
  • reduce the risk of falls or further decline
  • remain living safely in your own home
  • reduce pressure on a spouse, family member or carer.

Your services will be tailored to you. You may need one type of support or a combination of services from different members of our care and allied health team.

A plan based on your goals

At the beginning of your service, we will talk with you about what you would like to achieve. Your goal may be to shower independently, walk safely around your home, prepare a simple meal, manage medical equipment or return to an activity that is important to you. We will develop a clear plan with you and review your progress during the service. You remain involved in decisions about your care and can choose which supports you wish to receive.

How long does restorative care last?

The Restorative Care Pathway provides intensive, short-term support for up to 16 weeks. The length and type of support you receive will depend on your assessed needs and goals. Some people may only require assistance for a few weeks, while others may benefit from a longer period of coordinated care. Before your restorative care finishes, we will discuss your progress and any ongoing support that may help you continue living safely and independently.

Who is restorative care for?

Restorative care may be suitable if you are an older person who:

  • has experienced a recent decline in strength, mobility or confidence
  • is recovering from an illness, injury or hospital stay
  • is finding some everyday activities more difficult
  • could improve or maintain function with short-term nursing or allied health support
  • wants to remain independent and active at home
  • may otherwise need a higher level of ongoing care.

You do not need to have an existing Support at Home classification or be a current Capecare client. However, you will need an aged care assessment and approval for the Restorative Care Pathway.

Restorative care in the comfort of home

Wherever possible, Capecare delivers restorative support in your own home. This allows our team to understand how you move around your home, complete daily activities and use any equipment. We can then provide practical support that fits your environment, routines and preferences. Receiving care at home can also make recovery feel more manageable and reduce the need for unnecessary travel or extended stays away from familiar surroundings.

Working with your health professionals

Restorative care is most effective when everyone involved in your care works together.

With your consent, Capecare can communicate with your GP, hospital team, physiotherapist, specialist or other health professionals. This supports a safer transition home and helps ensure your services are coordinated around your health needs and goals.

How do I access restorative care?

Eligibility for the Restorative Care Pathway is determined through an aged care assessment. You may be referred following a hospital stay or by a health professional, or you can contact My Aged Care to request an assessment. Capecare can explain the service, discuss whether it may suit your circumstances and help you understand the next steps.

Restorative care in Busselton and surrounding communities

Capecare provides community aged care and restorative support across Busselton, Dunsborough and surrounding South West. Our local nursing, allied health and care teams work together to provide responsive, practical support that helps older people recover and remain independent at home.

Case study 

Short-term assistance supporting a safe recovery at home

A local elderly man sustained a neck fracture and was required to wear a supportive neck brace during their recovery.

The client’s wife was able to assist with day-to-day personal care but needed additional support to safely change the neck brace and replace and clean the brace pads. Without this assistance, managing the brace at home would have placed additional pressure on the client and his wife during an already challenging period.

He had not previously been receiving Capecare Community services.

The WA Country Health Service physiotherapy team initiated a referral for short-term restorative support following discussions with Capecare Community’s Service Support Officer and clinical team.

Through this entry-level funding, Capecare provided assistance for up to eight weeks while the client recovered. The support enabled the brace to be managed safely and correctly at home, while reducing the physical and emotional demands placed on the client’s wife.

The client was also offered physiotherapy support to help improve his mobility and assist him to return to his previous level of function. He chose not to take up this service at the time, reflecting his right to decide which supports best meet his needs and preferences.

This timely, short-term assistance allowed the client to remain in the comfort of his own home while recovering, without requiring an ongoing Support at Home classification or care package. It has also provided practical reassurance to his wife, knowing that trained support is available for an aspect of care she could not safely manage alone.

The support enabled the client to remain safely at home during recovery and reduced the pressure on his wife during a challenging period.
