Eligibility and Assessment

Eligibility

Capecare’s community services are provided through a range of government subsidised community services programs. The application process for each program is as follows:

Home and Community Care  (Including Day Respite)
You will need a referral from the Regional Assessment Services (RAS) Team for the Community Program to be deemed eligible to receive the services. To be considered eligible to be a client you should still be living in the community and have an ongoing functional disability.  Clients are able to contact Capecare Home Care Services directly.

Home Care Packages Level 1 & 2
To receive a Level one or two Home Care Package, you must be assessed by an Aged Care Assessment Team (ACAT) as requiring at least low level care. If a Package is available, and you would prefer to be cared for in your home, then you may be offered a package.
Information on ACATs is available from doctors, hospitals and community centres or My Aged Care on 1800 200 422 or at www.myagedcare.gov.au.

Home Care Packages level 3 & 4)
To receive a Level 3 or 4 Home Care Package, you must be assessed by an Aged Care Assessment Team (ACAT) as requiring at least high level care. If a Package is available, and you would prefer to be cared for in your home, then you may be offered a package.

Veterans Home Care
All veterans of the Australian defence forces who have a Gold or White Repatriation Health Card and their war widows/widowers may be assessed for services.  You do not automatically receive services by having a Gold or White Card. Services are provided on the basis of your assessed needs, available program resources and other factors.
You can be referred for an assessment by your doctor or other health professional, or you can ring your regional Veterans’ Home Care agency directly on 1300 550 450.

The Commonwealth Government has developed a number of information sheets detailing further information on what someone can expect when receiving Community Care.

Capecare can also offer private home care services, which do not require eligibility assessment, and can be arranged at an agreed fee.

Assessment

Each client receives an individual and comprehensive assessment prior to commencing services to ensure the most appropriate service is provided.

There are charges associated with each of the service types and the amount charged depends upon the person’s ability to pay, with no one denied services if they are not able to afford the payment. Capecare can also offer private home care services, which do not require eligibility assessment, and can be arranged at an agreed fee.

This is discussed in detail at the time of assessment.

Our highly professional Care Coordinators and Carers are fully trained and qualified to meet the care needs of the aged in their own homes to ensure that the very best possible standards of service are delivered at all times.

Capecare Community Services place great emphasis on preserving the privacy and dignity of all care recipients and all care arrangements are kept in the strictest confidence.

Assistance provided by these programs can include

  • Gardening and home maintenance

    Inclusive of lawns, assistance with [minor] house repairs, wood chopping etc.

  • Transport

    Shopping, appointments, banking and social outings

  • Domestic assistance

    Cleaning, laundry, meal preparation

  • Personal Care

    Medication prompts, hygiene needs (showering, dressing), welfare checks

  • Respite for Carers

    At the Wellness Centre for the main carer or client

  • Allied Health Services

    At the Wellness Centre for the main carer or client

  • Nursing

    Wound care, etc by qualified Registered / Enrolled Nurses

  • Day Center activities

  • Care assessment and reviews by our Client Coordinator
