With attractive modern facilities and staff that are renowned for excellence in care, the demand for our services is very high.
This means that there are waiting lists for most services at Capecare.
Preparing for our services
To ensure we can be of assistance as soon as places become available, it’s important you are accessed by an Aged Care Assessment Team (ACAT). You can contact an ACAT directly, through your Doctor, or your local hospital can provide a referral to the ACAT nearest you.
Once you have been assessed as eligible for residential aged care (permanent or respite), you will receive a copy of the completed assessment.
You should also ensure you receive an information pack, titled, ‘5 Steps to Entry into Residential Aged Care’ from the ACAT officer.
The pack contains an information booklet, application form and request for an assets assessment.

Whether you are applying for a permanent or respite place. Capecare will require a copy of the following documents before your name can be entered on the placement list:
a) A copy of current ACAT
b) A signed copy of completed application form
c) A copy of letter from Centrelink / Dept Veterans’ Affairs with your assets assessment or if you choose not to have an assets assessment, a letter advising the care facility.

Once you have forwarded or delivered the documents to Capecare, we look forward to discussing the intended care you require.
